In Singapore, diabetes is one of most common chronic diseases with a prevalence rate of 13% and one of the top 6 causes of death in 2015. A central focus in management of type 2 diabetes is glycemic control. In this study, we aim to determine optimal treatment policies in terms of timing and sequence of medication initiation for glycemic control for patients with type 2 diabetes in Singapore. In analysis, glycated hemoglobin (HbA1c) is used as a measure of blood glucose. Our study patients are newly diagnosed type 2 diabetic patients at National Healthcare Group polyclinics of Singapore in 2007 and patients are followed up for 8 years.  A Markov decision process (MDP) model has been developed to determine optimal treatment decisions of medication initiation for controlling glycelevel over patients’ lifetime.  In the model, health states are described by a discrete set of clinically relevant ranges of HbA1c levels. To lower hyperglycemia, three medications are considered in the study. Treatment decisions are made on a three-month basis from age 40 to 100. Patients’ quality adjusted life years (QALYs) are used as the reward in the MDP model. We compare model-derived treatment strategy with current protocol in Singapore. Using the developed model, preliminary numerical analysis demonstrates the improvement on expected QALYs compared with the current practice. The proposed model would facilitate evidence-based clinical decision making process and help clinicians to make informed and comprehensive treatment decisions for patients with type 2 diabetes.
